Survey Request: eHealth Questionaire by May 31

The AmCham is conducting its eHealth Questionnaire for the Czech Republic 2013. You are welcome to deliver your answers and improve the knowledge of eHealth environment. The results of the survey will be delivered to Ministry of Health Care. The survey consists of 7 questions. All answers will be confidential and results will be used only in aggregate. We would greatly appreciate if you would 1) complete the survey as soon as possible and 2) sending/asking  your business partners to also complete it. The deadline for completing the survey is May 31.
